stolen personal information. More than 60,000 Tennessee Benefits
Administration employees who took part in a 2013 wellness screening were
notified that their personal information may have been accessed after a
third-party vendor, Onsite Health Diagnostics, reported a breach of its
computer systems. The vendor stated that the breach took place between January
and April and that a new computer system with new securities was put in place.
